Who is eligible for coverage?
Any eligible self-employed person, employees and their dependents who become paid members of the National Business Network, which is a national business organization and social association.  One membership per company is required, if more then one person (employee) enroll in the program.

What rate class do I use?
Rate class is determined by your contribution to the plan.  If you are one person company, you use Rate Class A.  If you have other employees who enroll, then use the Rate Class by what percentage you contribute to their premiums.

When should I enroll my dependents?
Eligible dependents must be enrolled within 31 days of the date the dependent becomes eligible (enrollment date of Member, birth or adoption).

What are the medical underwriting requirements?
There are none during the open enrollment period.

Where is the plan available?
No preferred provider network or networks are required.  Participants may see the eligible provider of their choice.

Are pre-existing conditions covered?
Yes, from the effective date of coverage.

When does coverage begin?
When your enrollment forms are received by our office, all the necessary forms completed, first months payment to ASI and NBN.  Enrollment forms received by the 25th of the month, will have an effective date of the first of the next month.

When does coverage end?
An insured Member’s coverage ends when the Member is no longer eligible, premiums are discontinued (subject to the grace period), when the policy terminates, or when the Member is no longer in good standing with National Business Network.

Who files the claims under my coverage?
Either the Member or provider can file the claim with Select Benefits Administrators at 800-497-3699 or health provider can get assignable benefits.

If I have questions regarding my enrollment forms, whom do I call?

How do I pay for my coverage?
All premiums for this program are collected by direct debit basis (Monthly Bank Draft) through ASI (Administrative Systems, Inc) who administers our premium payments.

When does my bank draft deductions begin?
Deduction starts once we receive your application and your policy has been approved.  If you have made the first months premium, with your application, 30 days after the effective date of coverage, depending upon your effective date.

If I change my address, need a duplicate policy, file a claim, or have general questions whom do I call?

Who is the insurance carrier, brokerage firm,and administrator?
Symetra Life Insurance Company and Select Benefits Administrators is the company that insures the benefits and pays the claims that you submit.
